Physiotherapist - Intermediate Care at Home

We are seeking a qualified Physiotherapist to join our team in Stockport NHS Foundation Trust, working within our Intermediate Care at Home and Discharge to Assess service. This is an exciting opportunity to work in a dynamic and supportive environment, providing holistic multidisciplinary interventions and support to patients in their own homes or community settings.
Main DutiesThe successful candidate will be responsible for working as part of a multi-professional team, incorporating physiotherapists, occupational therapists, nurses, social workers, and rehabilitation support workers. The role will involve assessing patients who are clinically optimised and do not require an acute hospital bed, but may still require care services with short-term, funded support to be discharged to their own home or another community setting.
The Physiotherapist will be expected to work independently, but also as an integral member of the multidisciplinary team, providing person-centred care and support to patients with complex needs and problems. The role will require the ability to communicate effectively with patients, families, and other healthcare professionals, and to work under pressure in a fast-paced environment.

Person SpecificationThe ideal candidate will have a DipCOT/Degree in Physiotherapy, be registered with the Health and Care Professions Council, and have a UK driving licence. They will also have experience of working as part of a therapy team and multidisciplinary team, and be able to demonstrate sound clinical reasoning, excellent time management, and advanced communication skills.
The successful candidate will be required to undergo an enhanced DBS check and join the DBS update service. They will also be expected to work independently and as part of a team, and to be able to function effectively in a fast-paced environment.
