Discharge to Assess Rotational Physiotherapist

We are seeking a highly skilled and motivated Physiotherapist to join our Discharge to Assess and Intermediate Care at Home teams. As a key member of our community-based service, you will play a vital role in supporting individuals to achieve their full potential and live independently in their own homes.
Key Responsibilities- Deliver holistic, patient-centered care to individuals in their own homes, focusing on rehabilitation and enabling them to achieve their goals.
- Work collaboratively with a multidisciplinary team, including nurses, occupational therapists, and other healthcare professionals, to ensure seamless care transitions and effective discharge planning.
- Conduct thorough assessments and develop personalized care plans to address the unique needs of each individual.
- Provide education and support to patients and their families to promote self-management and independence.
- Participate in quality improvement initiatives and contribute to the development of best practices within the service.
