We are seeking a highly skilled and motivated Physiotherapist to join our Discharge to Assess and Intermediate Care at Home teams. As a key member of our multidisciplinary team, you will play a vital role in delivering holistic support and interventions to improve the health and well-being of individuals in their usual residence.
About the RoleThe Discharge to Assess service is a free, short-term rehabilitation service for adults who have been assessed as medically stable and do not require hospital care. As a Physiotherapist, you will work flexibly across both Discharge to Assess and Intermediate Care at Home teams, delivering assessments and interventions in patients' homes or in a bed-based facility. You will be expected to liaise with colleagues across all sectors to facilitate discharge and work independently to plan care and implement from a patient-centred approach.
Key Responsibilities- Deliver holistic support and interventions to improve the health and well-being of individuals in their usual residence.
- Work flexibly across both Discharge to Assess and Intermediate Care at Home teams.
- Liaise with colleagues across all sectors to facilitate discharge.
- Plan care and implement from a patient-centred approach.
- Hons. Degree in Physiotherapy or Masters degree in Rehabilitation with a licence to practise Physiotherapy.
- State Registration with the Health and Care Professions Council.
- UK driving licence.
- Evidence of Continuous Professional Development maintained in a portfolio.
