Discharge to Assess Rotational Physiotherapist

We are seeking a highly skilled and motivated Physiotherapist to join our Discharge to Assess and Intermediate Care team. As a key member of our team, you will play a vital role in delivering high-quality care to patients in their own homes, facilitating timely discharges and improving health outcomes.
Key Responsibilities- Deliver holistic, patient-centered care to individuals in their own homes, utilizing a range of assessment and intervention techniques.
- Work collaboratively with multidisciplinary teams to develop and implement discharge plans, ensuring seamless transitions between care settings.
- Contribute to the development and maintenance of clinical guidelines and protocols, ensuring best practice is embedded in our service.
- Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions.
- Provide education and training to colleagues and students, promoting knowledge and skills development.
