Job Summary
Stockport NHS Foundation Trust is seeking a skilled Physiotherapist to join our Discharge to Assess team. As a key member of our multidisciplinary team, you will work with patients who require short-term rehabilitation services to prevent unnecessary hospital admission and facilitate timely discharges.
Main Responsibilities
The successful candidate will be responsible for delivering holistic support and interventions to patients in their usual residence. This will involve assessing patients' needs, developing personalized care plans, and working collaboratively with other healthcare professionals to ensure seamless transitions of care.
Key Skills and Qualifications
- Qualified Physiotherapist with a honours degree in Physiotherapy or Masters degree in Rehabilitation
- State Registration with the Health and Care Professions Council
- UK driving licence and ability to travel to patients' homes
- Excellent communication and interpersonal skills
- Ability to work autonomously and as part of a team
