Job Summary

We are seeking a qualified Physiotherapist to join our Discharge to Assess and Intermediate Care at Home Teams. As a valued member of our team, you will play a crucial role in supporting and facilitating safe discharge for individuals who are deemed medically optimised.

Key Responsibilities

  • Deliver community-based interventions, demonstrating a high level of holistic care both independently and as part of a team.
  • Liaise with colleagues across all sectors to facilitate discharge while also working independently to plan care and implement a patient-centred approach.
  • Work as part of a multidisciplinary team with a broad skill mix, including registered Nursing staff, Occupational Therapists, and Trusted Assessors.
  • Comply with the Trust's policies and procedures, including the use of personal protective equipment and infection control measures.

Requirements

  • Qualified Physiotherapist with a strong understanding of community-based care.
  • Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
  • Strong problem-solving and analytical skills, with the ability to think critically and make informed decisions.
  • Full driving licence and business insurance, with the ability to travel independently.
