Job Summary

We are seeking a qualified Physiotherapist to join our Discharge to Assess and Intermediate Care at Home Teams. As a valued member of our team, you will play a crucial role in supporting and facilitating safe discharge for individuals who are deemed medically optimised.

Key Responsibilities

  • Deliver community-based interventions, demonstrating a high level of holistic care both independently and as part of a team.
  • Liaise with colleagues across all sectors to facilitate discharge while also working independently to plan care and implement a patient-centred approach.
  • Work collaboratively with a multi-professional team consisting of highly skilled, registered Nursing staff, Occupational Therapists, and Trusted Assessors.
  • Contribute to the development and enhancement of seamless transfer of care from hospital discharge to usual residence.

Requirements

  • Qualified Physiotherapist with a passion for providing community-based interventions.
  • Ability to work independently and as part of a team, demonstrating a high level of holistic care.
  • Excellent communication and interpersonal skills, with the ability to liaise with colleagues across all sectors.
  • Independent travel with the use of your own vehicle, business insurance, and a full driving licence are essential.

DBS Check

An enhanced DBS check will be required for this role, at a cost of £41.90, which will be deducted from your first Trust salary. Additionally, it will be a condition of your employment that you join the DBS update service and pay the annual subscription fee of £13.
