We are seeking a highly skilled Physiotherapist to join our Intermediate Care at Home team at Stockport NHS Foundation Trust. As a Physiotherapist, you will play a vital role in providing high-quality patient care and contributing to the development of our community-based services.
Key Responsibilities- Assess and treat patients with a range of conditions, including those with complex needs and mobility issues.
- Work collaboratively with multidisciplinary teams to develop and implement individualized care plans.
- Provide education and support to patients, families, and carers to promote independence and self-management.
- Contribute to the development and implementation of clinical guidelines and policies.
- Participate in quality improvement initiatives and contribute to the evaluation of service effectiveness.
- HCPC registration as a Physiotherapist.
- Experience of working in a community-based setting, preferably in a similar role.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
- Ability to work independently and as part of a team, with a flexible approach to working hours.
- Knowledge of relevant legislation, policies, and procedures, including the Mental Capacity Act and Safeguarding Vulnerable Groups.
