Key Responsibilities

As a Community Health Specialist, you will be responsible for providing comprehensive care management to patients within their homes.

You will work closely with nursing and social care colleagues to ensure seamless care transitions between hospital and community settings.

Person Specification

We are looking for someone with excellent observational and reporting skills, who can demonstrate good IT skills and keyboard proficiency.

A valid driving license and access to a vehicle or viable alternative are also required.
