\About the Role
\This full-time position involves working within a knowledgeable experienced team, providing comprehensive supervision and training programmes with career progression opportunities within the clinical and managerial fields.
\Main Responsibilities
\- To provide high-standard patient care in hospital settings and patients' homes\
- To ensure a more seamless flow for patient care between hospital and community settings, enabling patients to be cared for in their own home environment\
- To provide supervision and support to more junior staff\
- To triage referrals to the community team
