Main Duties
  • To provide high-standard patient care in hospital settings and patients' homes
  • To ensure a seamless flow for patient care between hospital and community settings, enabling patients to be cared for in their own home environment
  • Provide supervision and support to more junior staff
  • Triage referrals to the community team
