Job Description

This is a Band 6 Community Physiotherapist role that requires experience and passion for delivering high-quality care within patients' homes. You will work in the Ipswich West Community Team, alongside nursing and social care colleagues.

The successful candidate will have extensive knowledge of working within the NHS, IT skills, and the ability to manage complex casework. A Diploma or 1st Level degree in Physiotherapy and current HCPC registration are also essential requirements.

Responsibilities
  • Provide high-standard patient care in hospital settings and patients' homes.
  • Ensure seamless care transitions between hospital and community settings.
  • Provide supervision and support to junior staff.
  • Triage referrals to the community team.
