NHS Physiotherapy Service Manager

This role requires a skilled Physiotherapist to join our Ipswich West Community Team as a Band 6 Community Physiotherapist, providing high-quality patient care in patients' homes.
About the RoleYou will be working closely with nursing, social care colleagues, and out-of-hours teams to ensure a seamless flow for patient care between hospital and community settings. This is a full-time position, with hours and pattern of work to be negotiated.
Main Responsibilities- Provide high-standard patient care in hospital settings and patients' homes
- Ensure a seamless flow for patient care between hospital and community settings, enabling patients to be cared for in their own home environment
- Provide supervision and support to more junior staff
- Triage referrals to the community team
