Role Overview:

We are seeking a dedicated and experienced professional to join our team as a Primary Care Network Community Health Worker. The successful candidate will play a key role in improving public health and making a positive impact on the lives of children and families within our network of practices.

This role involves working closely with patients, families, and carers to support their personalised care needs and ensuring access to appropriate vaccinations. The successful candidate will also participate in outreach activities to raise awareness and promote vaccination uptake among local communities.

Key Responsibilities:

  • Undertake responsibilities in line with practice demand for childhood vaccinations
  • Build and maintain relationships with patients and families, considering health inequalities and diversity
  • Participate in outreach activities to raise childhood vaccination awareness and uptake
  • Work closely with other health, social, and educational care professionals to identify and support patients

Requirements:

  • Experience of working in a community setting
  • Excellent communication skills (written and oral)
  • Clear, polite telephone manner
  • Ability to work under pressure

Estimated Salary: £22,000 - £32,000 per annum, depending on experience and qualifications. We offer a competitive benefits package, including generous holiday entitlement and opportunities for professional development.
