We are seeking a highly skilled Adult Welfare Coordinator to join our Learning Disabilities Team in Manchester. As a key member of the team, you will be responsible for providing specialist support to individuals with learning disabilities.

The ideal candidate will have experience conducting assessments, developing support plans, and collaborating with other professionals to ensure a coordinated approach. You will also provide guidance and support to individuals and their families, maintaining accurate case records along the way.

Responsibilities:
  • Conduct assessments to identify individual needs and support requirements
  • Develop and implement person-centered support plans
  • Collaborate with professionals to ensure a coordinated approach
  • Provide guidance and support to individuals and their families
  • Maintain accurate case records
Requirements:
  • Degree in Social Work from a recognised university
  • Registration with the Health and Care Professions Council (HCPC)
  • Experience working with adults with learning disabilities
  • Excellent communication and interpersonal skills
  • Ability to work under pressure and meet deadlines
