Care Coordinator for Adults

We are seeking a highly skilled Care Coordinator for Adults to join our team in North Manchester. This role involves working closely with clients, their families, and other healthcare professionals to deliver high-quality care and support to adults with complex mental health needs. The successful candidate will possess excellent communication and organisational skills, with the ability to work effectively in an integrated multi-disciplinary team environment.
About the Position:
This position requires the successful candidate to complete comprehensive assessments of clients' needs, coordinate care packages, and provide safeguarding support as needed. They will also be responsible for developing written reports, including those required under legal frameworks. In addition, they will have the opportunity to participate in the development of new initiatives and contribute to the growth of the team.
Requirements:
To succeed in this role, you must hold a social work qualification and be registered with the Health & Care Professions Council. A strong understanding of the Mental Capacity Act 2005 and the Trust Care Programme Approach (CPA) guidance is essential. Additionally, you will need access to transport to travel widely across the city. A competitive salary of £35,000 - £40,000 per annum, depending on experience, is offered.
