Adult Services Coordinator

Tripod Partners is seeking a skilled Social Worker to join our CAD Team in Bury Adult Care Services. This role involves ensuring the safety and wellbeing of vulnerable adults through comprehensive assessment and support.

About the Role

  1. Assess referrals to identify service user needs and develop strategies for support.
  2. Develop and maintain relationships with service users, their families, and carers.
  3. Liaise with healthcare professionals, including GPs, hospitals, and community mental health teams.
  4. Work closely with local authorities to access resources and services.
  5. Provide emotional support and practical assistance to service users.
  6. Document and record all interactions and outcomes.
  7. Attend meetings and contribute to case discussions.
  8. Maintain accurate records and report any concerns or issues.
  9. Collaborate with other departments to achieve seamless service delivery.

About You

You must possess a strong foundation in social work principles and practice. Your ability to build trust and rapport with service users is crucial.

Compensation and Benefits

  • A competitive salary: £55,000 - £60,000 per annum.
