Therapeutic Outreach Specialist

We are seeking a skilled Therapeutic Outreach Worker to join our team at Polaris Children's Services. As a key member of our team, you will play a vital role in providing support and guidance to children and families in need.
Key Responsibilities- Develop and maintain strong relationships with children, young people, and their families to understand their needs and provide tailored support.
- Work collaboratively with internal teams to ensure a comprehensive and coordinated approach to service delivery.
- Conduct assessments and develop personalized plans to address the needs of children and families.
- Provide guidance and support to children and families to help them access community resources and services.
- Participate in regular team meetings and contribute to the development of new initiatives and programs.
To be successful in this role, you will need:
- A strong understanding of child and family development, including the impact of trauma and adversity.
- Excellent communication and interpersonal skills, with the ability to build rapport with children, young people, and their families.
- Ability to work collaboratively as part of a team and contribute to the development of new initiatives and programs.
- Strong assessment and planning skills, with the ability to develop and implement personalized plans.
- Knowledge of community resources and services, with the ability to provide guidance and support to children and families.
