Rural Frailty Service Lead
About the Role:We are seeking a highly skilled and dedicated Rural Frailty Service Lead to join our team at Powys Teaching Health Board. This is a challenging and rewarding opportunity for a professional to make a real difference in the lives of patients living with frailty in North Powys.
The successful candidate will be responsible for leading the development and delivery of expert care to individuals with severe frailty, working closely with families and carers to ensure comprehensive support and guidance. As a key member of our Frailty Team, you will have the autonomy to coordinate care services, making clinical decisions and providing direction for the Primary Care and Community Frailty Team.
This is a unique opportunity to shape the development of Frailty Nurse-led services in North Powys, working in close collaboration with Public Health Wales, Health Champions, and Primary Care to improve health outcomes and enhance the experience of healthcare services for service users and their families and carers.
Main Responsibilities:
- Responsible for the continual development of the Frailty Team ensuring that service delivery is of a high professional standard in line with Health Board policies and procedures.
- Responsible for reviewing all policies in relation to the Primary and Community Care Frailty Nurse service.
- Develop plans to ensure those with additional complex needs are enabled to access health promotion initiatives and healthcare effectively and appropriately.
Requirements:
- Registered Nurse - current NMC registration.
- Relevant master's degree or equivalent level of knowledge, skills, and experience to academic level.
- Evidence of continuing professional development at degree level.
- Working knowledge of legislation relevant to the field.
- Knowledge or experience of the principles of person-centred planning processes.
- Knowledge or experience of the principles of clinical governance and how it affects professional practice.
- Knowledge or experience of the Equality and Diversity agenda.
- Knowledge or experience of the health inequalities faced by people with frailty.
- Knowledge or experience of Best Practice in relation to decreasing the health inequalities faced by people with severe frailty.
- Experience of working with people who have frailty.
- Post-registered experience working with people with frailty.
- Excellent consultation and liaison skills and ability to develop and deliver presentations and teach others.
Salary: £45,000 - £60,000 per annum (dependent on experience).
