About the Job

You will be working as part of a close-knit team to provide assessment, care planning, and coordination of care from a social perspective to adults accessing our service. With a caseload of diverse adults, you will be responsible for ensuring they receive tailored support and guidance.

The ideal candidate will have experience working within a multi-disciplinary setting and possess a solid understanding of NHS and Local Authority policies and procedures. You should also be comfortable with attending funding panels and commissioning care packages.

Main Responsibilities
  1. Conduct comprehensive Care Act assessments and reviews for individuals with intricate Health and Social Care needs.
  2. Assume a leadership role in promoting adult safeguarding best practices and conduct safeguarding investigations when necessary.
  3. Oversee the commissioning of suitable health and social care services for service users.
  4. Promote the utilization of Direct Payments in service user and carer care and support plans.
  5. Perform regular risk assessments to ensure vulnerable individuals remain secure.
  6. Engage in reflective practice sessions and pursue further training to improve your professional growth.
