About the Role

We are seeking a highly skilled Clinical Mental Health Social Worker to join our Leighton Buzzard Community Mental Health Team. As a key member of the team, you will provide high-quality social care services to adults experiencing mental ill-health.

The successful candidate will have experience of working within a multi-disciplinary environment, with a strong understanding of the social determinants of mental illness and interventions to support recovery.

You will be responsible for assessing, planning, and coordinating care from a social perspective, ensuring that service users receive person-centred interventions.

In this role, you will work closely with acute inpatient units, the Crisis Team, Primary Care services, and other stakeholders to provide a recovery-focused model of care.

As a permanent employee, you can expect a competitive salary of £37,338 to £44,962 per annum, pro rata.

Key Responsibilities:

  • Liaise with acute inpatient units, the Crisis Team, and other stakeholders to provide a recovery-focused model of care.
  • Assess, plan, and coordinate care from a social perspective, ensuring person-centred interventions.
  • Work with service users to develop bespoke care plans, taking into account their unique needs and circumstances.
