Job Overview

We're seeking a highly skilled Advanced Nurse Practitioner to join our team at Central & North West London NHS Foundation Trust. As a key member of our primary care services, you will be responsible for delivering top-quality patient-centred care in a hospital setting, community clinic, or patient's own home.

Responsibilities
  • As an extended and supplementary prescriber, you will adhere to extant guidance and support patients in the use of their prescribed medicines or over-the-counter medicines.
  • You will contribute to practice targets (QOF etc.) by complying with local and regional guidance.
  • Liaise with external services/agencies to ensure patients are supported appropriately, particularly vulnerable patients.
  • Delegate clinical responsibilities appropriately, ensuring safe practice and tasks within the scope of the individual's practice.
  • Support the clinical team with all safeguarding matters in accordance with local and national policies.

Requirements
  • A job plan that demonstrates advanced nursing practice and equity with peers working at this level is essential.
  • You should have experience in prescribing and undertaking medication reviews.
  • Working in a primary care environment is also crucial.
