Community Mental Health Professional

Join East London NHS Foundation Trust as a Community Mental Health Professional, working in the Leighton Buzzard area. This rewarding role involves providing high-quality social care services to adults experiencing mental ill-health.
About the RoleIn this position, you will work as part of a multi-disciplinary team to provide assessment, care planning, and coordination of care from a social perspective to adults accessing our service. You will hold a caseload of adults with diverse mental health and social care needs and ensure that they receive comprehensive support and guidance.
You will also be responsible for liaising with acute inpatient units, the Crisis Team, Primary Care services, and the Voluntary and Private Sector to provide a recovery-focused model of care. As an integral member of our team, you will play an active role in the discharge of our service users from hospital and commission health and social care services as required.
To excel in this role, you must possess experience working within a multi-disciplinary environment and have a thorough understanding of the issues and policies within NHS and Local Authority environments. You should also be familiar with attending funding panels and commissioning care packages.
Key Responsibilities- Provide comprehensive assessments and reviews under the Care Act for individuals with complex Health and Social Care needs.
- Take a lead in promoting adult safeguarding practices and undertake safeguarding enquiries when necessary.
- Ensure appropriate health and social care services are commissioned on behalf of service users.
- Support the promotion of Direct Payments in service user and carer care and support plans.
- Maintain regular risk assessments to ensure vulnerable people are kept safe from harm.
- Attend reflective practice sessions and access post-qualifying training to enhance your professional development.
