About the Role: Young Persons and Adult Link Worker
We are seeking a compassionate and dedicated individual to join our team as a Young Persons and Adult Link Worker. In this role, you will play a vital part in connecting individuals with essential resources and support services within our community.
Your Responsibilities:
- Provide guidance and support to young persons and adults seeking assistance with various needs.
- Connect individuals with appropriate community resources, including healthcare, housing, education, and social services.
- Maintain accurate records of client interactions and referrals.
- Collaborate with other professionals and organizations to ensure a coordinated approach to service delivery.
- Contribute to the development and improvement of our support programs.
