Youth Support Specialist

The position of Young Person's Practitioner (Risk and Resilience) presents a remarkable opportunity to have a meaningful impact on the lives of children and young individuals (C&YP) who face significant risks such as child exploitation, substance abuse, or potential absences from home.
In this role, you will provide customized support and guidance to children, young people, and their families, contributing to the achievement of high standards of care, quality, and positive outcomes. Your efforts will empower service users to reach their full potential through consistent and impactful engagement.
Key Responsibilities:
Delivering personalized one-on-one support, guidance, and interventions. Evaluating the needs of children and young people, formulating suitable support plans and pathways, and informing risk mitigation strategies while facilitating access to additional or specialized services as needed. Ensuring that referrals, support, care, and risk management processes are of exceptional quality, facilitating smooth transitions. Cultivating sustainable collaborative work with referring agencies, local authorities, children's social care, and family services. Maintaining timely and accurate data recording, including tracking, outcome monitoring, KPI assessment, and assisting in the creation of feedback reports. Offering consultation, information, training, and presentations to a diverse range of stakeholders from various settings, with opportunities to enhance your skills in training delivery if desired.This role involves collaboration with a wide array of partner agencies and commissioners, providing ongoing opportunities to expand your network and expertise within the Children's Social Care Sector.
