Job Title: Community Care Coordinator

We are seeking an enthusiastic and organized individual to join our Intermediate Care Teams as a Community Care Coordinator in Dorset. As a member of our supportive multidisciplinary teams, you will play a vital role in promoting independence and improving quality of life for adult patients living in their own homes and Care homes.

About the Role:

Your key responsibilities will include providing admin support to ensure the efficient day-to-day running of the service, including providing rehab-based care to people in their own home, including a degree of personal care in the short term. You will also work closely with other team members to monitor deteriorating patients with support from clinicians and contribute to the delivery of high-quality patient care across 7 days.

As a Community Care Coordinator, you will have the opportunity to develop your skills and knowledge in a dynamic and supportive environment, with access to excellent learning and development opportunities, including the chance to apply for Nursing, Physiotherapy, and Occupational Therapy Apprenticeships.

Responsibilities:
  • Providing rehab-based care to people in their own home, including a degree of personal care in the short term.
  • Monitoring deteriorating patients with support from clinicians.
  • Collaborating with other team members to achieve service goals.

The salary for this role is approximately £28,000 - £35,000 per annum, depending on experience, plus weekend enhanced pay.
