Job Overview

We are seeking a highly skilled and dedicated individual to join our Home Haemodialysis team as a Haemodialysis Care Coordinator. This is an excellent opportunity to work in a dynamic and supportive environment, providing high-quality care to patients with renal conditions.

Main Responsibilities
  • To provide comprehensive support to patients undergoing haemodialysis at home, ensuring they receive the highest level of care and attention.
  • To work collaboratively with the multidisciplinary team, including Renal Nurse Specialists, to develop and implement patient-centred care plans.
  • To train patients on haemodialysis techniques, providing ongoing education and support to ensure they feel confident and empowered in their treatment.
  • To conduct regular follow-up visits, taking blood specimens, troubleshooting and resolving any issues that may arise.

This role requires excellent communication and interpersonal skills, with the ability to build strong relationships with patients, colleagues and other healthcare professionals. You will need to be flexible, able to travel off-site and work independently when required.

Salary and Benefits

We offer a competitive salary package, including a basic salary of £35,000 - £45,000 per annum, depending on experience. Other benefits include access to a car allowance, paid annual leave, pension scheme and opportunities for professional development and training.
