Job DescriptionAs a Support Practitioner, you will be working directly with individuals to provide support and encouragement, enabling them to live independently.
You will be part of a dedicated team providing person-centred care, focusing on each individual's unique needs and goals.
This is an excellent opportunity for those looking to start a career in care or develop their existing skills and knowledge.
The role requires a strong ability to build rapport and establish trusting relationships with clients, as well as excellent communication and problem-solving skills.
You will work closely with colleagues and external partners to ensure seamless delivery of services and support.
