About the Role

We are seeking an experienced social worker to join our team as a Care Coordinator for Adults in Buckinghamshire. As a key member of our team, you will work closely with adults, their families, and other professionals to understand their needs and wishes.

You will be responsible for managing a caseload, providing a high-quality professional service, and ensuring regular monitoring and review of care and support plans. Your excellent communication and interpersonal skills will enable you to build strong relationships with service users, their families, and other stakeholders.

Main Duties and Responsibilities
  • Develop and implement strength-based plans that meet the needs and wishes of adults and their families.
  • Apply relevant statutory frameworks to promote independence and autonomy while balancing risks and the needs of others.
  • Manage a caseload and provide a high-quality professional service within a framework of appropriate supervision.
  • Collaborate with other professionals to ensure comprehensive support for adults in Buckinghamshire.
  • Ensure compliance with legislation, statutory guidance, and local policies and procedures.
  • Monitor and review care and support plans to ensure they meet the needs of service users.

The estimated salary for this position is £37,338-£44,962 per annum, pro rata. We offer a competitive benefits package and opportunities for career development.
