We are looking for a highly skilled Social Work Specialist in Adult Services to join our team in Wycombe, Buckinghamshire. The ideal candidate will have a strong background in adult social work, excellent communication skills, and the ability to work effectively in a fast-paced environment. Responsibilities include delivering targeted services, conducting assessments, and promoting the wellbeing and safety of vulnerable adults.
Responsibilities:
• Deliver high-quality social work services to adults with complex needs
• Conduct comprehensive assessments and develop individualised care plans
• Foster positive relationships with service users, families, and other stakeholders
• Maintain accurate records and contribute to the evaluation of service effectiveness
• Participate in multidisciplinary teams to achieve shared goals
