Virtual Ward Triage Clinician

Job Summary
Norfolk Community Health and Care NHS Trust is seeking a skilled Virtual Ward Triage Clinician to join our team. As a key member of our Virtual Ward Hub, you will provide clinical triage and ongoing management of referrals, working closely with clinicians across Norfolk and Waveney.
Main Duties
- Assess and prioritize incoming referrals, determining the level of care required and processing referrals accordingly.
- Contribute to the delivery of high standards of healthcare to patients by overseeing and responding to remote monitoring systems.
- Communicate regularly with patients and their carers, triaging clinical concerns and presentations as they arise.
- Develop knowledge and skills in SystmOne ledger management to ensure awareness of Virtual Ward demand and capacity.
- Participate in multidisciplinary meetings and contribute to the requisitioning of supplies and equipment.

Professional Requirements
- Registered Adult Nurse Qualification
- Diploma or 1st level degree
- Current NMC registration
- Evidence of continuing professional development

- Experience of working within a multidisciplinary team
- Experience of working with patients with multiple pathologies and delivering care plans to meet their needs
