Job Summary

We are seeking a compassionate Care and Support Practitioner to work with a young adult with Autism in her own flat, providing transitional support to learn essential life skills and increase confidence and independence.

About the Role

This rewarding role involves assisting the individual with daily living tasks, such as meal preparation, medication management, attending appointments and activities, managing personal finances, and providing prompts with personal care. You will also take an active interest in their hobbies and support them in exploring new opportunities in their local community.

Responsibilities
  • Provide empathetic and caring support to enable the individual to live independently.
  • Assist with daily living tasks, including meal preparation, medication management, and personal care.
  • Support the individual with hobbies and interests, such as jigsaw puzzles and watching movies.
  • Encourage and facilitate participation in community activities and events.
  • Maintain accurate records of the individual's needs and progress.
Requirements

To be successful in this role, you will need:

  • An empathetic and caring nature.
  • Excellent verbal and written communication skills.
  • Confidence in using computers.
  • The ability to work well in a team environment and on your own.
  • Good organisational skills.
Shift Patterns

This role includes day shifts lasting 12-13 hours, commencing at either 8am or 9am based on the day's scheduled activities, as well as Waking Night shifts from 8pm to 9am.

£25,000 - £28,000 per annum, depending on experience (approximate salary range).
