Support Practitioner for Young Lady with Autism and Epilepsy

About the Role
We are seeking a motivated and understanding Support Practitioner to join our team and support a young lady with Autism and epilepsy in her own flat. The successful candidate will have a full UK, manual driving licence with a minimum of 1 year's driving experience, as the young lady will be getting her own mobility car.
Key Responsibilities
- Provide transitional support to help the young lady learn and develop essential life skills, including looking after her own home, increasing her confidence and independence.
- Assist the young lady with all aspects of daily living, including meal preparation, medication management, attending appointments and activities, managing personal finances and providing prompts with personal care.
- Take an active interest in the young lady's hobbies and support her in exploring new opportunities in her local community.
Requirements
- An empathetic and caring nature.
- Excellent verbal and written communication skills.
- Confidence in using computers.
- Ability to work well in a team environment and on your own.
- Good organisational skills.
Shift Patterns
The role will include day shifts lasting 12-13 hours, commencing at either 8am or 9am based on the day's scheduled activities, as well as Waking Night shifts from 8pm to 9am.
