Beelieve Health and Wellbeing Services is seeking a Support Practitioner to provide high-quality care and support to individuals in their own homes and communities.

Key Responsibilities:
  • Provide 1:1 support to young adults and elderly individuals, assisting with daily living tasks and promoting independence.
  • Work closely with the management team to deliver direct support as outlined in individual care plans.
  • Maintain accurate records of support, reviews, and risk assessments.
  • Contribute to staff team supervision and appraisals, ensuring a positive and supportive work environment.
Requirements:
  • A confident driver with a valid license, able to travel between services.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and colleagues.
  • A commitment to delivering high-quality care and support, with a focus on promoting independence and well-being.
