We are currently seeking a Relief Support Practitioner to join our team in our Community Outreach service.

About the Role

As a Relief Support Practitioner, you will play a vital role in supporting individuals with their everyday lives, enabling them to live as independently as possible.

You will work closely with colleagues, families, and individuals to deliver high-quality care and support, fostering a team-oriented approach.

Responsibilities will include:

  • Assisting with healthcare needs, including medication administration and personal care
  • Promoting independence and personal growth by following Support Plans tailored to each individual's needs and goals
  • Maintaining accurate and up-to-date records of daily tasks and activities
  • Working collaboratively with colleagues, families, and individuals to deliver high-quality care and support
