About the Role

We are seeking a highly skilled and experienced Primary Care Mental Health Practitioner to join our team at Bradford District Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality mental health services to our patients.

Key Responsibilities
  • Provide specialist assessment and treatment of individuals with mental health problems in a primary care setting.
  • Develop and implement holistic care plans that address the physical, emotional, and social needs of patients.
  • Work collaboratively with other healthcare professionals to ensure seamless transitions of care and effective communication.
  • Provide education and training to patients, families, and carers on mental health issues and self-care strategies.
  • Contribute to the development and implementation of new and innovative models of care within primary care and secondary mental health services.
Requirements
  • Registered Professional in either Nursing (Mental Health), Social Work or Occupational Therapy, with current registration with NMC, Social Work England or HCPC.
  • Educated to degree level or equivalent experience.
  • Previous experience of working with individuals with serious mental health problems, including assessment, care planning, and risk management.
  • Experience of working collaboratively within a multidisciplinary team and contributing to service development and quality improvement.
