About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Specialist to join our team at Bradford District Care NHS Foundation Trust. As a key member of our Older People's Mental Health Liaison Service, you will play a vital role in supporting patients in acute Trust ward areas and transitioning between hospital and care home settings.

Key Responsibilities
  • Act as a resource clinician, providing support and clinical advice to patients and healthcare professionals.
  • Ensure implementation of all Trust policies and procedures, promoting clinical excellence and best practice.
  • Develop and deliver training programs to enhance the skills of healthcare professionals in acute hospital settings.
  • Assess and address complex mental health issues, working collaboratively with multidisciplinary teams to provide comprehensive care.
  • Contribute to the planning, monitoring, and control of resources, ensuring effective use of resources and achieving agreed standards of care.
  • Apply the Mental Health Act and maintain knowledge of relevant policies and procedures.
  • Ensure equality of opportunity and fair access for all service users and carers, promoting a culture of respect and empathy.
  • Develop and maintain effective communication networks, collaborating with other disciplines, services, and agencies to ensure continuity of patient care.
Requirements
  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker with 2 years of experience in a relevant setting.
  • Evidence of continued professional development and a commitment to ongoing learning.
  • Understanding of the needs of older adults with mental health problems and experience in assessing complex issues within an interdisciplinary framework.
