About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Bradford District Care NHS Foundation Trust. As a key member of our multidisciplinary service, you will play a vital role in supporting older adults with mental health needs in acute hospital settings.

Key Responsibilities
  • Provide clinical advice and support to patients, families, and healthcare professionals.
  • Assess and develop care plans for older adults with mental health needs.
  • Collaborate with multidisciplinary teams to ensure seamless care transitions.
  • Develop and deliver training programs to enhance clinical skills and knowledge.
  • Contribute to the development and implementation of service policies and procedures.
  • Participate in quality improvement initiatives to enhance patient outcomes.
Requirements
  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker with 2 years of experience in a relevant setting.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Knowledge of older adult mental health and ability to assess complex issues within an interdisciplinary framework.
