We are seeking a highly skilled and experienced Mental Health Liaison Specialist to join our team at Bradford District Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to older adults with mental health needs.
Key Responsibilities- Act as a resource clinician, providing support and clinical advice to patients, families, and healthcare professionals.
- Ensure the implementation of all Trust policies and procedures, promoting a culture of excellence and best practice.
- Develop and deliver clinical care and services, working collaboratively with multidisciplinary teams to achieve optimal outcomes.
- Receive and assess referrals, making informed decisions about the most appropriate course of action, and referring patients to other professionals as needed.
- Work closely with clients and carers, encouraging participation in care planning and delivery, and promoting empowerment and self-management.
- Contribute to the planning, monitoring, and control of resources, ensuring effective use of resources and minimizing waste.
- Ensure agreed standards of care are achieved, maintaining a focus on quality and safety.
- Apply the Mental Health Act, maintaining knowledge of and adhering to Directorate policies and procedures.
- Ensure equality of opportunity and fair access for all service users and carers, promoting a culture of inclusivity and respect.
- Take a lead role in ensuring that issues of clinical governance, research, audit, and good practice are addressed within the service.
- Develop an effective communication network, working collaboratively with other disciplines, services, and agencies to ensure continuity of patient care.
- Ensure care delivery is in accordance with relevant professional guidance and current legislation.

We are looking for a candidate with the following qualifications and experience:
- Registered Mental Health Nurse, Occupational Therapist, or Social Worker.
- 2 years' experience in a relevant setting.
Desirable qualifications and experience include:
- Teaching, training, and/or supervision of clinical staff.
- Experience of working in general and mental health hospital settings.
