About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Specialist to join our team at Bradford District Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality care and support to older adults with mental health needs.

Key Responsibilities
  • Act as a resource clinician, providing support and clinical advice to patients, families, and healthcare professionals.
  • Ensure the implementation of all Trust policies and procedures, promoting a culture of excellence and best practice.
  • Develop and deliver clinical care and services, working collaboratively with multidisciplinary teams to achieve optimal patient outcomes.
  • Receive and assess referrals, making informed decisions about patient care and coordinating interventions as needed.
  • Work closely with clients, carers, and healthcare professionals to promote participation in care planning and delivery.
  • Contribute to the planning, monitoring, and control of resources, ensuring effective use of resources and achieving agreed standards of care.
  • Apply the Mental Health Act and maintain knowledge of relevant policies and procedures.
  • Ensure equality of opportunity and fair access for all service users and carers.
  • Take a lead role in addressing clinical governance, research, audit, and good practice within the service.
  • Develop an effective communication network, collaborating with other disciplines, services, and agencies to ensure continuity of patient care.
Requirements
  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker with a minimum of 2 years' experience in a relevant setting.
  • Evidence of continued professional development and a commitment to ongoing learning.
  • Understanding of the needs of older adults with mental health problems and knowledge of assessing complex issues within an interdisciplinary framework.
