We are currently recruiting for a skilled Adult Social Worker to join our client's Hospital Discharge Team at Newport City Council. Key Accountabilities- To work effectively with multi-disciplinary teams to coordinate patient care.
- To engage with service users, their families, and carers to understand their needs and preferences.
- To develop and maintain accurate records of case work, including assessments, plans, and progress reviews.
- Degree-level qualification in Social Work or equivalent.
- Post-qualified experience in adult social care.
- Social Work England registration.
- A salary of £38,000 - £45,000 per annum, based on experience.
