Social Work Job Opportunity: Up to £35 per hour

Hoop Social Work is recruiting for a Social Services Specialist to join our team in a hospital setting. This role offers the chance to work with adults over 18 years with physical disabilities and older people.

The ideal candidate will have a strong understanding of the Social Services and Well-being Act and be able to complete assessments in line with this legislation. They will also need to be confident in applying the principles of the Mental Capacity Act and able to identify triggers for CHC.

The discharge team works closely with the multi-disciplinary team within the hospital, including A&E and SAU to prevent unnecessary admissions. The successful candidate will work closely with MDT colleagues and use an outcome-focused, person-centered approach to complex case work.

To succeed in this role, you will need:

  • A minimum of 6 months experience of working in the UK as a registered social worker
  • A relevant Social Work Qualification
  • Registration with Social Care Wales / Social Work England
  • Enhanced DBS, registered on the update service or be happy to apply for a DBS
  • 3 Years of referencing
  • Driving license with access to a car
