About the Role

Your responsibilities will include conducting thorough assessments, identifying potential barriers to discharge, and developing personalized care plans to facilitate seamless transitions. You will collaborate closely with healthcare professionals, individuals, and families to deliver high-quality patient-centered care.

To excel in this role, you will need to possess:

  • A relevant Social Work Qualification
  • Registration with either Social Care Wales or Social Work England
  • An Enhanced DBS
  • Minimum 6 months' experience working in the UK as a registered social worker
