Competitive Salary: £35 per hour

We are seeking a highly skilled Care Professional to join our discharge team at Hoop Social Work. This is an exciting opportunity to work with adults over 18 years with physical disabilities and older people.

The successful candidate will have a strong background in social services, with the ability to assess individuals in a fast-paced hospital environment. You will need to be confident in applying the principles of the Mental Capacity Act and able to identify triggers for CHC. Knowledge of using the WICCis system would be helpful in this role.

The discharge team works closely with the multi-disciplinary team within the hospital, including A&E and SAU to prevent unnecessary admissions. As a member of the team, you will work closely with MDT colleagues and use an outcome-focused, person-centered approach to complex case work.

To apply for this role, you will need to hold:

  • A minimum of 6 months experience of working in the UK as a registered social worker
  • A relevant Social Work Qualification
  • Registration with Social Care Wales / Social Work England
  • Enhanced DBS, registered on the update service or be happy to apply for a DBS
  • 3 Years of referencing
  • Driving license with access to a car
