As a seasoned Adult Hospital Discharge Social Worker, you will be working closely with the multidisciplinary team at one of our partner hospitals. This is an exciting opportunity to make a difference in the lives of adults with physical disabilities and older people.

About the Role

The successful candidate will hold a relevant Social Work Qualification, registration with either Social Care Wales or Social Work England, and an Enhanced DBS. With a minimum of 6 months' experience working in the UK as a registered social worker, you will have a strong understanding of the Mental Capacity Act and CHC triggers.

This is a challenging yet rewarding role, requiring you to work collaboratively with individuals, families, and healthcare professionals to facilitate safe and effective hospital discharges. You will be responsible for conducting assessments in line with the Social Services and Well-being Act, identifying potential barriers to discharge, and developing personalized care plans to ensure seamless transitions.

We are looking for an individual who is passionate about delivering high-quality patient-centered care, has excellent communication skills, and can work effectively in a fast-paced environment.

A competitive salary of £35 per hour (Umbrella company) awaits the successful candidate, along with opportunities for professional growth and development.
