Job Summary
The Isle of Wight Council's Adult Social Services department is seeking a dedicated Adult Social Services Worker to join our Wellbeing & Access Hub. As an Adult Social Services Worker, you will play a vital role in providing essential support to individuals at the first point of contact with Adult Social Care. You will work closely with Outreach, Carers IW, and Occupational Therapy colleagues to help reduce long-term care needs by arranging support through reablement services. About the Role
In this role, you will be responsible for conducting timely assessments that are strength-based, supporting planning that promotes and maintains independence, choice, and autonomy of individuals. You will also implement and promote departmental safeguarding procedures, adhere to departmental recording policy, and attend multi-agency discharge planning meetings to ensure timely discharges. Additionally, you will support carers and family members, sharing and advising when necessary, as well as offering Carers Assessments in line with the Care Act 2014. To succeed in this role, you must have a passion for promoting people's strengths and the ability to make a difference in social work practice. A suitable candidate should be suitably qualified, hold up-to-date professional registration, demonstrate knowledge and understanding of The Care Act 2014, Mental Capacity Act 2005, Data Protection Act 1998, and evidence of ability to work according to practice guidance/procedure. A salary of approximately £25,000 per annum is offered, plus benefits including annual leave entitlement of 32 days p/a PLUS public holidays, Local Government pension scheme, and various other perks such as discounted Solent ferry travel and cycle to work scheme. An estimated salary range for this role is £22,500 - £28,000 per year.
