We are seeking a skilled and compassionate Rehabilitation Social Worker to join our Hospital Social Work Team on the Isle of Wight. As a member of our team, you will play a vital role in supporting individuals who have been discharged to Rehabilitation beds.
The successful candidate will have a social work qualification and be registered with the relevant professional body. They will also have experience of working in a fast-paced environment and promoting a person-centred and strengths-based approach.
Key Responsibilities:
- To work as part of a multi-disciplinary team to support individuals who have been discharged to Rehabilitation beds.
- To assess the needs of individuals and develop care plans that promote their independence, choice, and autonomy.
- To implement and promote departmental safeguarding procedures and adhere to departmental recording policies.
- To attend multi-agency discharge planning meetings to ensure timely discharges.
Requirements:
- Social work qualification and registration with the relevant professional body.
- Experience of working in a fast-paced environment.
- Ability to promote a person-centred and strengths-based approach.
- Knowledge and understanding of The Care Act 2014, Mental Capacity Act 2005, and Data Protection Act 1998.
