Hospital Social Worker - Newport Job Title

Hop Social Work seeks a highly skilled and experienced Hospital Social Worker to join our team in Newport. As a member of our dynamic team, you will be responsible for providing high-quality social work services to patients in need of hospital discharge planning and coordination.

You will be required to work closely with multidisciplinary teams, including medical professionals, nurses, and other social workers, to ensure seamless patient care and effective discharge planning.

Responsibilities and Duties
  • Conduct comprehensive assessments of patients' needs and develop individualized plans to meet those needs.
  • Collaborate with healthcare providers to coordinate patient care and ensure timely discharge from the hospital.
  • Provide education and support to patients and their families to promote healthy behaviors and self-care.
Requirements
  • A bachelor's degree in social work or a related field.
  • A minimum of 2 years of experience working in a hospital setting as a social worker.
  • Registration with Social Care Wales / Social Work England.
  • Enhanced DBS, registered on the update service or willing to apply for a DBS.
