Job Title: Diabetes Specialist Nurse

We are seeking an experienced Diabetes Specialist Nurse to join our team at East Lancashire Hospitals NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and supporting individuals with diabetes to manage their condition effectively.

About the Role

This is an exciting opportunity for a skilled and motivated nurse to work in a dynamic and supportive environment. As a Diabetes Specialist Nurse, you will be responsible for providing expert advice and support to patients, their families, and other healthcare professionals. Your primary focus will be on empowering patients to take control of their diabetes management, promoting independence, and improving health outcomes.

Key Responsibilities
  • Deliver patient-centered care, assessing individual needs and developing personalized care plans.
  • Provide education and support to patients, their families, and carers, promoting self-management and independence.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Stay up-to-date with current developments in diabetes care, adhering to national guidelines and best practices.
  • Contribute to the development and implementation of education programs, promoting awareness and improving standards of diabetes care.
  • Participate in shared responsibility for stock management and reordering as necessary.
  • Act as an autonomous practitioner, providing expert advice and support to patients and healthcare professionals.
Requirements
  • Registered Nurse Level 1
  • Degree module in Diabetes management or equivalent relevant experience
  • Non-Medical Prescribing Qualification (willing to undertake)
  • Car driver/car user essential
  • Ability to develop colleagues and effectively impart knowledge to others
