About the Role

We are seeking an experienced and skilled Senior Diabetes Specialist Nurse to join our team at East Lancashire Hospitals NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and supporting individuals with diabetes across our services.

Key Responsibilities
  • Deliver patient-centered care and support to individuals with diabetes, their families, and carers, in line with National Service Framework (NSF) and National Institute for Health and Care Excellence (NICE) guidelines.
  • Conduct advanced assessments of patient needs and provide expert advice on diabetes management, empowering individuals to attain independence in self-management.
  • Develop and implement structured educational programs for patients, families, and carers, promoting effective diabetes care and management.
  • Collaborate with healthcare professionals to develop a holistic support network for individuals with diabetes, ensuring seamless care and coordination.
  • Participate in shared responsibility for stock management and reordering as necessary.
  • Provide evidence-based, proactive nursing services, responsive to user needs, and delivered through face-to-face consultations.
  • Act as an autonomous practitioner, providing expert and advanced specialist advice and support to patients, families, and healthcare professionals.

Requirements
  • Registered Nurse Level 1
  • Degree module in Diabetes management or equivalent relevant experience
  • Non-Medical Prescribing Qualification (willing to undertake)
  • Car driver/car user essential
  • Ability to develop colleagues and effectively impart knowledge to others
